Social Work
At NSHN, our Social Worker is an integral part of the multidisciplinary team, offering support to inpatients in crafting and executing personalized plans for their care, rehabilitation, and eventual discharge.
If you would like to speak with the Social Worker during your admission, please ask a member of your care team to complete a referral.
Assessments
Interviews with patients, family members, Secondary Decision Makers (SDM), Power of Attorney (POA), caregivers and community partners
Consults with the multidisciplinary team and liaise with community agencies
Identify barriers to discharge, individual needs and help navigate systems
Discharge Planning
Links to programs, services, community resources
Completes referrals as required throughout hospital admission to discharge
Facilitation of family meetings/conferences
Alternate Level of Care (ALC)
Inform patients, SDMs and POAs of ALC process, next steps, required documents and relevant copayment when applicable
Complete appropriate referrals as required
Assists with system navigation to obtain required documents, financial information and other required information
Advocacy
Work with patients, family members, SDM, POA, caregivers, community partners and interdisciplinary team to access required supports and services
Consults with community services to facilitate support
Maintaining knowledge and dialogue regarding barriers or trends in community and possible impact to facilitating services
Education
Advise patients, family members, SDM, POA and caregivers of available services, required assessments and referrals, relevant eligibility requirements, potential wait times for services, ALC process, system navigation and other resources or trends.
Jenna Murray
Social Worker
Jenna is a registered Social Worker, with a Masters of Social Work degree. Jenna has advanced training in mental health, addictions, intimate partner violence, child development, the criminal and family justice system and Geriatrics. Jenna graduated from Sault College in 2013 with a Social Services Worker diploma, then continued her studies at Algoma University where she obtained her Honours Bachelor of Social Work degree in 2016 and in 2017 obtained her Masters of Social Work degree from the University of Windsor. Jenna is a registered Social Worker with the Ontario College of Social Workers and Social Service Workers. Jenna understands that asking for help and talking with a stranger can be overwhelming. Jenna’s person-centered approach means that she works alongside you to work with you with where you are at. Jenna uses a collaborative approach as she believes every person is an expert in their own lives and needs individualized support. The therapeutic approaches she uses include trauma informed, solution focused, client centered and cognitive behavioural therapy. In her spare time, she loves to be outside, workout, travel, spend time with her golden retriever and spend quality time with her family and friends.
